Laser treatment of periodontal pockets - (APDT) Antimicrobial Photodynamic Therapy


The periodontitis is an almost unnoticeable and not self-stopping process, progressing in some years inevitably to loosening and then to loss of teeth and implants. Generally the patient learns too late, that she or he is facing tooth loss.

There are different risk factors in the evolving and progress of periodontitis, loosening of teeth in case of every patient. The risk factors of loosening and tooth loss are to each analyzed and learned for each patient individually.

After tooth loss, if the missing tooth has been replaced by a dental implant, and the risk factors were neither fully known, nor sufficiently altered, - the original risk factors resulting in periodontitis and tooth loss, would further exist as implant risk or implant loss risk, and with great probability the implants would face the same destiny as the lost natural teeth.

 

Laser surgery of periodontal pockets (eradication of bacteria in the pockets)

The epidermis and the full bacterial flora of the gingival pockets around the teeth or implants can be eliminated by short impulse laser beam, advancing circularly in depth and on the surface of the pockets. This operation is carried out with surgical diode lasers, at the generally used 2,5-3W output, with 200-300 micrometre flexible glass fibres after plaque removal. According to the therapy’s protocol, laser surgery of periodontal pockets for all of the teeth and implants, for the whole mouth cavity is performed on the first, eighth and fifteenth day. The microbes can be eliminated regardless their kind, or drug resistance. It is an up to date, efficient treatment, with minimal discomfort, without bleeding operation, the most efficient therapy for periodontal pockets.

Soft laser therapy (promotion of healing and gum attachment)


The continuous soft laser therapy is complementary to the oral hygienic treatment and laser surgery of periodontal pockets. This therapy can be carried out partly by oral hygienists at the clinic, daily, or every second day, partly by patients at home, after training, with commercially available soft laser devices. 

Antimicrobial photodynamic therapy with laser (APDT)

The red soft laser light of 650-660 nm wave length, applied together with methylene blue solution is capable to eradicate microorganisms in pockets, on inflamed areas, with similar efficiency to surgical lasers, in mild cases.
When applied after surgical lasers, the soft laser completes its effect, destroys the micro-organisms recolonizing to the periodontal pockets from the other mucous membranes of the oral cavity, from the tonsils, from the sinuses. The new small (pencil) size soft laser is very powerful; the duration of the therapy can be considerably shortened (15 sec per point with 200 mW laser, earlier the duration with 20 mW laser was ten times more). With use of the new powerful laser the price of the therapy has also considerably decreased, what is a further advantage.


Administration of special medical mouth rinse inhibiting periodontitis

The amount of bacteria can be reduced by plaque removal, by surgical lasers, but the regular treatment with disinfectant on the other oral mucosa surfaces and in the pharynx promotes the reduction of bacteria in the full mouth. The clinical disinfectant mouthwash developed by the Gáspár Medical Center for periodontal therapy is proved to be very efficient.


Hyaluronic acid gingival and periodontal pocket therapy

The hyaluronic acid gel developed for clinical use, with higher hyaluronic acid concentration completes very efficiently the therapies, when administered into periodontal pockets, on inflamed soft tissues, after surgical interventions due to its favourable effects on inflammation of the gum, wound healing, bacteria reduction. The clinical hyaluronic acid gel, hyaluronic acid toothpaste and mouth spray developed by the Gáspár Medical Center for periodontal therapy proved to be very efficient.
